Keep in mind the Death Certificate Index for Blount County is not comprehensive. In the early days, up through the 1920s, not all the deaths in Blount County were reported to the county or the state. Additionally, many Blount County residents died in hospitals in Birmingham (neighboring Jefferson County) and their deaths will be recorded in that county. For one reason or another, many deaths were simply unreported and researchers have to use other sources such as Federal Census, Bible records, and newspapers to capture the information.
However, this list should capture the majority of the deaths occurring in Blount County between 1908 and 1929. Keep in mind this list is an index only. Major libraries across the state have the microfilm images of the actual certificates. In some cases, the certificate includes date and place of birth, parents, mother's maiden name, and place of burial. Unfortunately, many times this information is missing, particularly in the earliest death certificates.
Also, many names are spelled phonetically or were indecipherable when compiled into this listing. Every attempt was made to edit this list to comtemporary standards of spelling to aid the researcher. If you detect an error, please let me know so I can amend the list. Since this list is an index only, and most certainly contains some errors, the actual death certificate should serve as the ultimate documentation authority. If the microfilm is unavailable, certified copies can be ordered through any county health department by filling out a form and paying a fee of $12 for each request. It's the same form used to request duplicate birth certificates for passports and other legal documents. The health department will fax the form to Montgomery. Usually, you can get same day service on those requests.
Once you get a form, if you prefer, you can handle the request by mail. Mail requests sometimes take up to six weeks to get back. The address is:
Alabama Department of
Public Health
Center for Health Statistics
P.O. Box 5625
Montgomery, Alabama 36103-5625
For more information, or to request blank forms, call (334) 206-5418. For Alabama residents, visit a local county health department. The same form can be used to request death, birth, marriage, or divorce certificates.
